Government strategy has highlighted integrated care as necessary for service transformation to deliver better joined up care whilst reducing system-wide costs. Integrated care is not only important for frail older people, but also those with enduring mental health problems, long term conditions and children with complex needs (National Collaboration for Integrated Care and Support, 2013).
